Discover what cohealth services you can access through the National Disability Insurance Scheme (NDIS). NDIS is for anyone between the age of 7 years old and 65 years old who has a permanent and significant disability. If you are eligible and have an NDIS plan, NDIS will pay for services and supports in your plan.

What is NDIS

NDIS stands for National Disability Insurance Scheme. NDIS is for people who have a permanent and significant disability. The purpose of NDIS is to help you to get the support you need to live and enjoy life. cohealth is registered with NDIS and we can provide certain services that might be in your NDIS plan. cohealth is a registered NDIS provider

cohealth is one of many NDIS providers. We can help you if you already have an NDIS plan. We are a registered provider of the following supports:

  • support coordination – for people living in central, northern and western Melbourne
  • specialist support coordination – for people living across Victoria
  • Podiatry
  • Occupational Therapy

We welcome all people and have staff specialised in supporting people who are:

  • experiencing mental illness
  • living with a disability
  • experiencing homelessness or unstable housing
  • engaged in the criminal justice system
  • using drugs or alcohol
  • from a culturally or linguistically diverse background

How does it work

First, if you are eligible, you need to apply for the NDIS and get an NDIS care plan. cohealth can then provide you with services relating to your plan.

There are multiple steps involved in getting an NDIS plan. We have listed the main steps below to help you understand what’s involved and how it works.

The first step is to find out if you are eligible for NDIS. Visit the NDIS website to check if you are eligible.

If you are eligible, the next step is to make an appointment with a Local Area Coordinator (LAC). They are someone who can help you with the next steps including making an access request (see below for more information on an Access request) and creating an NDIS plan. A Local Area Coordinator can help you to make an access request for the NDIS. This will include providing NDIS with information from a health professional about your disability. Find out more about how to make an access request on the NDIS website.

A plan is something that is created by the NDIS in collaboration with you. The purpose of the plan is to work out what kind support and services you need to enjoy your life. Find out more about the process of creating an NDIS plan on the NDIS website. Once you get an NDIS plan approved, this means that NDIS will pay for services and supports in your plan.

If your NDIS plan includes support coordination, you will be able to choose cohealth to coordinate your support. This means that we’ll help you select your support services and help you get the best out of your NDIS package/plan.

Common questions about using NDIS to access cohealth services

Local Area Coordinators are people who can help you apply for the NDIS. Call the National Disability Insurance Agency on 1800 800 110 to make an appointment with a Local Area Coordinator who can help you apply.

You can also get help from a health advocate or a client advocate. A client advocate is someone who helps you get assistance with healthcare tasks such as making an appointment, or with filling out forms. If you are aged 16 years or older and have episodic or ongoing mental illness which has an impact on your psychosocial functioning, you may be able to get support to apply for the NDIS:

  • If you live in the North West of Melbourne, this service is provided by Neami. You can contact them on 1300052588 to find out if you are able to access this support.
  • If you live in the Western Region of Melbourne, this service is provided by Commonwealth Psychosocial Support (CPS) You can contact them on 9448 6880 to find out if you are able to access this support.
