Our Theory of Change shows how we work towards our vision of healthy people and healthy communities. It explains the impact we want to have and the broad approach we will take to get there.
It supports our 10-year strategy and guides the work we will do over the next decade, as we work towards our vision. We will continue to monitor and evaluate our progress, and look forward to sharing more stories of our impact in the coming years.
1. What we believe
We believe that:

- many people are left behind because of growing health problems, higher living costs, and a complex healthcare system — especially those who already face challenges to staying healthy and well.
