Family Violence and Child Information Sharing

Where necessary, cohealth will share information with other authorised organisations under the Information Sharing Schemes.

Under the Family Violence Information Sharing Scheme, cohealth prescribed information sharing and risk assessment entities will share information to assess and manage the risk of family violence. cohealth authorised services will also share information to promote children’s wellbeing and safety in accordance with the Child Information Sharing Scheme.

If you are an authorised information sharing entity and wish to make a request for information from cohealth, please complete the Family Violence and Child Information Sharing request form and forward it to our centralised information sharing intake team via incomingISS@cohealth.org.au
