National Relay Service (NRS)

cohealth uses the National Relay Service (NRS) to ensure our contact numbers are accessible to people who are deaf or have a hearing or speech impairment.

Access to medical records

You have a right to request access to your health information and ask for it to be corrected if necessary.

Please complete a Request for Access to Medical Records Form, available from reception at any cohealth service or download here. We may charge a fee (as allowed under legislation) for photocopying and sending the information.
