Our service aims to improve the health and wellbeing for people over 55 living in public housing in the City of Melbourne or Moonee Valley.

cohealth does this by providing access to various supports and services and help our clients improve their social and community involvement.

What do I need to do?

People over 55 can contact cohealth about our Active Connections Program directly or referrals can be made from external agencies.

What will it cost me?

There is no cost for Active Connections Program.
