cohealth mental health outreach provides Mental Health Community Support Services (MHCSS) to people experiencing mental illness across greater Melbourne.
Our focus is on personal recovery for both consumers and the people who are important to them.
We work with people experiencing mental ill health and those close to them, in their homes and community on parts of life they may find challenging. For example this may be to reduce social isolation, explore new or revisit old interests, or to develop skills and build confidence in daily living activities.
We also provide Personal Helpers and Mentors (PHaMs) and D2DL June 2016 programs from some of our outreach sites.
What do I need to do?
Each of our mental health outreach sites provides MHCSS and these need to be accessed by referral from a centralised intake & assessment:

What will it cost me?
There is no cost for eligible people experiencing a mental health issue and aged between 16-64 years in and around the areas we service.
