Our general practitioners are central to cohealth’s model of care, and in FY25 delivered 166,545 service occasions for our communities. They are often the first point of contact for clients, and play a vital role in delivering holistic, person-centred health services.
By building trusted relationships and addressing both immediate and long-term health needs, our general practitioners help improve health outcomes, reduce health inequities, and strengthen the wellbeing of our communities.
They are specialists in their own right, trained to treat the whole person and to care for people of all ages, all walks of life, and with all types of medical issues and concerns.
At cohealth, general practitioners work as part of a multidisciplinary team – alongside nurses, allied health professionals, mental health clinicians, and community workers – to provide integrated, person-centred care that addresses the full context of a person’s health and wellbeing.
They take time to listen, build relationships, and understand the broader factors impacting a person’s health, from chronic conditions to housing, trauma, and social isolation.
Our general practitioners also often act as connectors, linking clients with alternative and community-based care options that reflect cultural, social and individual needs — from traditional healing practices to social prescribing and community supports.
