cohealth is co-locating at Melton Health & Community Services. The new facility, which is also home to services offered by Djerriwarrh Health Services, Melbourne Health-Midwest Area Mental Health Service, MacKillop Family Services and Odyssey House Victoria, offers a holistic approach to client care with both mental and physical health interventions on offer.
cohealth has been delivering progressive mental health services from a single office site in Unitt Street, Melton for over 10 years. Now, this exciting new opportunity to co-locate with multi-disciplinary services in a new state of the art building takes service delivery to the next level.
Melton Health & Community Services will support some of the most vulnerable people in the community. Often people need more than one person or team involved in their care to manage their illness. The co-location enables cohealth’s mental health team to work collaboratively with allied health services, alcohol and other drug services and clinical mental health providers to bridge service gaps and provide a more holistic model of care.
Through the new facility, cohealth is providing a range of mental health programs including community support services, personal helper and mentors service, outreach support, peer led recovery groups and day to day living recreation groups.
The facility features consulting rooms, group rooms, a gym, training and assessment kitchen, as well as staff areas and a mental health tribunal room.
