who we are

We provide ‘care for all’ – offering culturally safe and inclusive services for people of all ages, genders, backgrounds and identities.

We understand that each person’s circumstances, needs, and goals are unique, and often change over time. That’s why we’re committed to meeting people where they are, offering flexible care that can respond to their health and social needs at every stage of life.

We know that certain population groups face greater challenges to being healthy and well.

The social determinants of health can influence up to half of an individual’s health outcome1. Almost one in three people living in areas of most disadvantage had two or more chronic conditions, compared to one in eight of those living in areas of least disadvantage2.

That’s why we specialise in delivering tailored responses that remove barriers and address the unique needs of these individuals and communities.

Whether it’s a local health centre, on the street, or in a local community space – we take our care wherever our clients need it.

Our centres offer everyone access to our holistic, accessible, integrated care, bringing together a broad range of health, wellbeing and social supports under one roof.

We also know that, for many in our communities, a physical building can be an obstacle to accessing healthcare. That’s why our outreach teams go directly to the people who need care. This can be an isolated engagement to address acute needs, or a pathway towards building trust and engaging more robustly with our care.

Additionally, our specialist services provide a range of responses for individuals with specific needs — for example, refugee and migrant, Aboriginal and Torres Strait Islander, and LGBTQIA+ communities and those experiencing higher instances of homelessness, addiction and mental illness.
