We have a long, proud history of partnering with our communities to provide equitable health and social care that best responds to their unique and evolving needs – this past year has been no different.
It has been a challenging time for many individuals and communities, with cost-of-living pressures continuing to be felt1, universal healthcare decreasing2, and inequality growing3.
Despite these challenges, our cohealth team has remained committed to ensuring all Victorians and Tasmanians can access high-quality care, regardless of their background or personal circumstance.
We’re proud to have delivered 526,500 service occasions to our clients in FY25, while continuing to champion the systemic change needed to improve health and wellbeing across our communities.
Innovation and partnership have been at the heart of our work in FY25, and our Impact Report showcases what we’ve achieved, together.
We’ve partnered to deliver innovative services and solutions, like the new Bourke Street health clinic in Melbourne’s CBD, and the Victorian Virtual Emergency Department at the Parkville Youth Justice Precinct.
We have expanded our outreach services in the CBD and western suburbs enabling us to connect with more people outside of the four walls of a clinic – people who may otherwise not connect with care.
We’ve partnered to provide health and social care at Make Room – an initiative supporting people to transition from homelessness to a secure, sustainable home.
We’ve connected Aboriginal and Torres Strait Islander community members with culturally safe health and social care, and we’ve empowered families to manage asthma at home, reducing the need to present at the emergency department.
Our achievements in the past year reflect what’s possible when innovation meets strong, purpose-driven partnerships.

It’s a source of immense pride that despite many challenges, cohealth has remained true to its mission of improving health and wellbeing for all.
