Transferring your TIMC Records to a new Medical Practice
Please ask the reception at your new Practice for a copy of their Transfer of Medical Records form and ask them to email the completed form to: reception@integratedmedical.com.au for actioning. There is no cost for transferring your records to your new General Practice.
Access to your TIMC records
You will need to download and complete a Request for Access to Health Records Form (PDF 126 kb), also available from reception at any cohealth service. We may charge a fee (as allowed under legislation) for photocopying and sending the information. Our fee structure aligns to the Health Records Act 2001, which can be viewed on the Health Complaint Commissioners website.
Cost of transferring medical records
Costs may be associated with your request in line with the Health Records Act 2001. When we contact you, we will let you know if there are any costs. Full details of costs available at the Health Complaints Commissioner website.
Subpoena or Summons
As a result of the significant number of subpoenas received, please ensure that you have included the relevant information below to enable cohealth to promptly comply.
A subpoena requiring production of TIMC health records should include:
- Client’s full name;
- Date of birth;
- That the client attended The Integrated Medical Centre
- The date or timeframe when the client attended the service.

How to Service a Subpoena or Summons
When serving a subpoena on cohealth, ensure you provide reasonable notice (preferably two weeks), to enable cohealth to comply.
In order to comply and be processed by cohealth, the subpoena must be:
- Stamped by the Court;
- Addressed correctly;
- Include sufficient conduct money; and
- Delivered to cohealth by 4pm on or before the last date of service
If permitted by the applicable court rules, you can serve a subpoena or summons for production of documents via email to: privacy@cohealth.org.au

Attendance to give evidence
As The Integrated Medical Clinic has closed, we are unable to assist with former staff members attending to give evidence.
Coronial Requests
Requests from the Coroner for TIMC client’s records should be emailed to privacy@cohealth.org.au
Requests from Lawyers or Insurance Agents
All requests for client records must be made in writing and include a signed consent from the client or their legal guardian, completed within the 12 months prior to the request.
Requests should clearly state the following information:
- Client’s full name;
- Date of birth;
- That they received care from The Integrated Medical Centre; and
- The date or timeframe when the client attended The Integrated Medical Centre.
Where possible, cohealth will confirm consent with the client prior to actioning the request.
cohealth will charge in line with the current period’s fee’s a listed on the Health Complaint Commissioners (HCC) website. A list of the current period’s fees is available on the HCC’s website. If you would like to request that fee’s be waved, please include this as part of your request letter.
