safer care victoria compassionate care award

Program: Mobile Health Access Point Program (MhAP)
Partners: Youth Support and Advocacy Service (YSAS) North Western Melbourne PHN and Green Cross Project Inc
About the program: The MhAP is a multi-disciplinary assertive outreach program that uses a modified bus as a mobile health clinic, where young people receive immediate Alcohol and other Drug (AOD), medical and other support.

secretary’s award for excellence in culturally diverse health

Program: Human relations education program for newly arrived young people
Partners: Women’s Health West (lead agency), Centre for Culture, Ethnicity and Health, Maribyrnong City Council, Western English Language School
About the program: The Human Relations program is a five-week program that prepares newly-arrived refugee and migrant young people for their transition from the Western English Language School (WELS) into mainstream schooling.

chief health officer’s award for supporting healthy populations

Program: Community Champions Take Action
Partners: Women’s Health West, City of Melbourne, Djerriwarrh Health Services, HealthWest Primary Care Partnership, Hobsons Bay City Council, Inner North West Primary Care Partnership, Melton City Council, Moonee Valley City Council, Victoria University, Western Bulldogs Community Foundation
About the program: The Community Champions Take Action supports women and their children to lead safe and healthy lives by challenging conditions that cause and maintain inequity and injustice.
