If you or someone you are caring for is Aboriginal or Torres Strait Islander and has trouble accessing services, this access and support program can help link you to services, and support you to remain living at home.

This program may be appropriate if you or someone you care for:

  • Has diverse needs or circumstances
  • Needs help to understand the range of support services that may be available
  • Needs help to access services

Fees & eligibility

Aboriginal and Torres Strait Islander people aged 50 and over who eligible for Commonwealth Home Support Program funding.

Aboriginal and Torres Strait Islander people aged 50 and under who have a disability and are eligible for HACC Program for Younger People funding (and their carers).
