Aboriginal and Torres Strait Islander people of any age with a chronic health condition can receive help to access health services and community groups and programs.
Aboriginal and Torres Strait Islander people can access a care coordinator who is a registered nurse to assist in navigating the service system. The care coordinator can also help with the management of long-term health conditions.
Our Aboriginal staff ensure that all the right medical providers and health professionals are on board and have the right information to help our clients. This allows them to concentrate on their health.
