cohealth Care Finders deliver assertive, one‑to‑one outreach for older people who are homeless or at risk of homelessness and disconnected from services, supporting navigation of My Aged Care, assessment preparation, paperwork completion, service connections, and ongoing check‑ins while working alongside health, housing, and community providers to reduce barriers and disengagement.
People aged 65 years and over, or Aboriginal and Torres Strait Islander people aged 45 years and over, who are homeless or at risk of homelessness.
