cohealth’s Indigo program provides support and care coordination for people aged 16 years or over who are experiencing any combination of mental illness, homelessness, disability, drug or alcohol abuse, or with current or previous experience of the forensic system.

By taking the time to understand the history, needs and issues of each of our clients, we work to find solutions with clients and service providers that aim to help clients improve their living conditions, health, wellbeing and feel safe and connected.
The Indigo program is a state-wide assessment and care plan coordination service. This approach has been developed to respond to the needs of people eligible for the Victorian Government’s Multiple and Complex Needs Initiative (MACNI), TAC and NDIS to assess and coordinate care plans with clients and service providers. We aim to help clients with multiple and complex needs receive the best service possible from their service providers.
Training for service providers
We also work with service providers to build their skills to better understand each client’s personal needs and how best to work with them for optimal outcomes.
