cohealth is a registered NDIS provider for Support Coordination, Occupational Therapy, and Podiatry for people aged 17 years or over who are experiencing any combination of mental illness, homelessness, disability, drug or alcohol abuse, or with current or previous experience of the forensic system. We also provide Complex Care Coordination under TAC MACNM. 

 

Service details

By taking the time to understand the history, needs, and issues of each of our participants, we work to find solutions with participants and service providers that aim to help participants improve their living conditions, health, and wellbeing, and feel safe and connected. 

A cohealth Support Coordinator will support participants in understanding their NDIS plan, identifying the most suitable service providers, and working to develop the skills they need to independently manage services in to the future. We aim to help participants with multiple and complex needs receive the best service possible from their service providers.

Fees & eligibility

  • Eligible participants with a NDIS plan with funding for support coordination, occupational therapy, or podiatry.
  • Participants aged 17 years or over who are experiencing any combination of mental illness, homelessness, disability, drug or alcohol abuse, or with current or previous experience of the forensic system.
