Senior Connections hosts welcoming and active social group sessions for people over 65. We offer social support up to five days a week across various locations in Carlton, Glenroy, East Keilor, Niddrie and Strathmore Heights.

Senior Connections offers opportunities for older adults who may require additional supports to spend time with others, making positive social connections.

Our groups support participants to:

Our services can provide transport to those within the allocated catchment area, morning tea, lunch, and light movement activities. Our activities include regular excursions, arts and crafts, story sharing and social time with peers and staff.

Our staff are trained aged care support workers from diverse backgrounds and experiences. We offer in-language support and interpreters at each of our sessions to ensure participants can fully engage.

Our social groups are developed and reviewed based on community need and can be targeted for specific cultural groups, general groups or high-needs groups. All participants receive an individual goal-directed care plan, which is reviewed annually or as needed.

Fees & eligibility

People aged over 65 with a My Aged Care referral or Home Care Package living in City of Melbourne, Monee Valley or Glenroy area. Talk to us about how to get an My Aged Care Referral.

If you are not registered for My Aged Care yet contact them for an assessment on 1800 200 422. Ask your assessment officer to refer you to cohealth Senior Connections. If you have a Support at Home package contact us directly by phone on 03 9448 5509, or by email on senior.connections@cohealth.org.au

A co-contribution fee for registered My Aged Care clients is requested for each session. Fees are under $20 and includes lunch, transport with catchment areas, activities, excursions and staffing. Fees can be discussed at your onsite assessment.

Clients on Home Care Packages will have a daily cost of $150 charged to your package provider.
