This service aims to improve the health and wellbeing for people over 55 who live in public housing in the City of Melbourne or City of Moonee Valley. We provide access to various supports and services and help our clients improve their social and community involvement. This helps them to maintain independence and stability.
Service details
Active connections workers provide low level monitoring and one to one outreach support to older people living in public housing.
One to one support varies but can include:
- Support with tenancy issues
- support to maintain independent living
- Referral to other health and support services
The program also holds group activities onsite, as well as excursions for older residents living at the low-rise public housing at Hall Street and Pascoe Vale Road.
