Community health aims to improve health and wellbeing for everyone in a community. We provide high quality health and support services, community programs and groups, as well as outreach services.

Community health is a specific approach to healthcare. It aims to help individuals and families improve their health by taking into account their environment and social conditions. As a community health provider, cohealth offer a broad range of services.

What is community health?

The idea behind community health is to help people live healthy lives by considering the whole person. A person’s mental health, physical health, social connections and environmental circumstances are all interconnected. This means the approach to improving their health and wellbeing needs to be interconnected as well. Community health means that many different health and support services all work together as an integrated whole.

Community health is for everyone

Community health providers like cohealth offer affordable health care, in particular for people who have low income. Health Care Card holders are also offered a discounted fee or can access some services for free.

Types of community health services

cohealth offers lots of different community health services. On our website, they are grouped into three broad categories.

Medical, dental and allied health are all integral to maintaining and improving health and wellbeing. Health services include things like doctors, physiotherapists, dietitians and dentists. Examples of support services include counsellors, social workers and homelessness support services.

Group activities and community programs are important for social connection and wellbeing. We have a range of different community groups that you can participate in. They include things like:

  • group exercise classes to help maintain physical wellbeing
  • education sessions to learn from health professionals about topics like healthy eating
  • social clubs and events for building social connection supporting mental health

Outreach services proactively reach out to people in their own home or environment and are vital for removing barriers to accessing healthcare. These services don’t require you to visit a cohealth location. Instead, our services come to you. The purpose of outreach services is to make it easier for you to access the service you need by removing the barrier of travelling to a cohealth location.

Examples of outreach include:

  • a mobile doctor service that sets up a mobile clinic in your area
  • a health professional that visits you at your home

cohealth is a not-for-profit community health organisation. We provide essential health and support services in Melbourne’s CBD, inner-north and inner-west, and the east coast of Tasmania.
