Commonwealth Home Support Program (CHSP)

cohealth is a registered provider of Commonwealth Home Support Program (CHSP) funded allied health, nursing and social support services. CHSP supports older people who need some help to stay independent at home and in the community. 

You may be able to get cohealth services funded by CHSP if you are needing some help to stay independent at home and: 

• are aged 65 years or over; or  

• are Aboriginal or Torres Strait Islander and aged 50 years or over. 

The first step in getting CHSP is getting registered for an assessment with My Aged Care. Go to the registration section at the bottom of this page to find out how. 

Once you have been assessed for and are eligible to receive CHSP, you can speak to your aged care assessor about sending a referral to cohealth for your CHSP services.

Support at Home

A Support at Home (S@H) package also supports older people to live independently in their home and community by providing coordinated support and services. 

You can access cohealth services through your S@H package. If you are approved for S@H, you or your care provider can arrange these funded services by contacting cohealth. cohealth will need your provider’s approval to deliver funded services to you.  

The first step in getting S@H is getting registered for an assessment with My Aged Care. Go to the registration section at the bottom of this page to find out how. 

If you are an older person who wishes to use funded cohealth services to support your independence at home or in the community, you will need an aged care assessment. There are three ways you can do this.   

• You can apply online on the My Aged Care website.  You will receive a call back to discuss your eligibility.  

• You can apply over the phone by calling My Aged Care on 1800 200 422  

• You can apply in person.  If you prefer to speak with someone face-to-face contact My Aged Care at 1800 227 475 and ask to make an appointment with an Aged Care Specialist Officer.

The first step will be to test your eligibility for the funded aged services.  If you are eligible, an assessment organisation will call you (or someone you choose) to discuss your needs and, based on the discussion, arrange a date for the assessment.  Assessments are carried out in your home by a trained assessor. They can also be arranged at other locations or via telehealth when necessary.

CHSP

If you have been assessed and given a CHSP, you will be provided with some My Aged Care referral codes. These codes are to be used to organise services with cohealth.

If you have a My Aged Care referral code for Allied Health services, contact cohealth’s Service Access on 03 9448 5521 to book an assessment.  

If you have My Aged Care referral code for Social Support or Respite, contact cohealth on 03 9448 5509 to learn more about services provided. 

S@H

Being approved for a package does not mean you will be allocated a package right away.  It means you are on a wait list for a package.  In the meantime, you will be able to access CHSP services. 

Once you have been allocated a package, you will need to tell your Care Partner that you would like to use your package to receive services from cohealth . Please let us know if you have been receiving services from us and have been allocated a package while you are receiving care.
