- are on a community correction order
- are on parole with a Mental Health and Rehabilitation condition.
Service details
Our team is made up of mental health clinicians, a GP, and care coordinators, who can:
- support clients with their short and long-term mental health and recovery goals
- develop a care plan with each client
- help clients get access and referrals to a range of health and social supports.
The team offers each client in the program up to eight sessions, which can take place at corrections facilities in the northwest region of Victoria, or at another venue that is suitable for the client.
