HARP provides support to clients who have long-term health conditions. We provide coordinated care to better manage your condition and prevent hospital admission.
The hospital admission risk program (HARP) is provided in partnership with The Royal Melbourne Hospital, cohealth, Merri Health and Bolton Clarke (formerly the Royal District Nursing Service).
The program provides information and support to people with long-term chronic health conditions. If you have a chronic health condition and are overwhelmed with your health care needs we can help manage your condition.

We work with clients to establish sustainable community supports and link them with appropriate services like medical clinics, education programs and exercise programs.
Adults with long-term chronic health conditions
If you have recently been admitted to hospital, or are at risk of being admitted to hospital in the City of Moreland, Moonee Valley and City of Melbourne (i.e. Royal Melbourne Hospital or other hospital) you can ask your doctor to make a referral.
A medical referral via your Doctor can be made via the Direct Access Unit at Melbourne Health (Ph 8387 2333).

People in other metropolitan regions of Melbourne should contact their primary hospital (eg Western Health about HARP, St Vincents HARP, Northern Health, Austin Health or Alfred Health) to find out more about their local HARP services.
There is no cost for this service.
