Do you want to help shape cohealth’s brand? Are you available to take a survey between Wednesday 7 November 2018 and Friday 9 November 2018?

We are looking for 30 people to say what they think about cohealth’s brand. Fill out this registration form for your chance to take part. The form should take about 5-10 minutes to finish and has 19 questions.

We will use the information gathered to select a range of participants to go through to the next stage. If you are chosen, you will be sent a survey link via email which should take about 20 minutes to finish. You will also get a $20 Coles Myer gift card for finishing the second survey.

The survey results will help tell us about cohealth’s brand in the future.

Registration closes on Tuesday 6 November 2018 (Melbourne Cup Day).
