• people aged 18 and over with Type 2 diabetes who have been either newly diagnosed or have not been accessing diabetes care already
• people aged over 18 and over who would benefit from assessment and diabetes education from relevant allied health professionals.

Speak to your GP or current cohealth clinician about a referral to the diabetes clinic.

External referrals can be submitted using our referral form by selecting ‘diabetes educator’ and listing reason for referral as ‘diabetes clinic’. Referrals can also be made via fax at 03 9034 8180

Please include:

• Reason for referral
• Medical summary
• Current medication list
• Latest Blood test results including HbA1c, Lipids, LFTs, UEC, Vit D, TSH etc

N.B. If client is 65 years and over, please complete a MAC referral for Allied Health.
